2. Photo Release/Consent Form

   The content submitted through this form may be used in various community materials, including newsletters, social media, websites, and promotional content. By sharing your photos, you help us showcase the unique and wonderful aspects of our community.

   By submitting a photograph or video (the "Work") through this web page, you irrevocably grant to Anthem Community Council, Inc., its licensees, and assigns (collectively, “ACC”) a non-exclusive, perpetual, payment-free, residual-free, and royalty-free world-wide, all-media license to use all or any portion of the Work, and also to distribute, reproduce, modify, adapt, display, sublicense, and incorporate all or any portion of the Work into other projects, postings, news story, article, communication or other works created by the ACC (“ACC Created Materials”) and to use the Work in any medium or format or through any other technical process whether now known or hereafter devised and to use the Work in any promotion, advertising, or marketing of ACC Created Materials. You expressly release ACC from any and all claims arising out of the use of the Work by ACC, including but not limited to moral rights or claims for blurring, distortion, or editing. You also agree to waive any equitable remedies against ACC for its use of the Work. You are over the age of 18. If the Work depicts a minor, you either (1) are the custodial parent or legal guardian of all minors depicted in the Work, or (2) have permission to submit the video from the custodial parent or legal guardian of all minors depicted in the Work. 

   You agree that the ACC may give you attribution for your Work, but is not required to so. You acknowledge that you have submitted the Work voluntarily and have no expectation of privacy or confidentiality with respect to the Work, and that no fiduciary relationship exists between the ACC and you or any other party based on the Work. The ACC may store your information, but makes no guarantees to destroy or archive the Work.

   Please only send JPEG files that are under 5 MB in size and only 3 files at a time.
